Welcome New and Returning Blue Devils!
Registration for the 2025-2026 school year is currently open. For detailed information and instructions please
click here. Fees for the 2025-2026 school year will be $200, plus $17 for Chromebook insurance.
Proof of Residency will be required for the following groups of students:
- All incoming freshmen
- All families who used an affidavit last year
- All families whose lease expires/expired between August 1, 2024 and September 1, 2025
- All families who have moved within District 121 boundaries

Returning Students
**If your student attended WTHS at either campus previously and you have left District 121 and are now returning, please contact your campus registrar before continuing.**
Returning families must:

In PowerSchool, complete the online registration forms A - I.

Provide proof of residency within District 121 boundaries if:

You are an incoming freshman
-
You used an affidavit to register last year
- Your lease expires/expired between August 1, 2024 and September 1, 2025

You have moved within District 121 boundaries.

Provide a copy of your student’s health and immunization records if:

You are an incoming freshman

You are a senior and need to provide proof of a second meningitis vaccination.

You need to provide a sports physical.

Provide a copy of any court paperwork/custody arrangements (if applicable).

New Transfer Students

PLEASE READ ALL THE INFORMATION BELOW BEFORE CLICKING THE NEW STUDENT ENROLLMENT BUTTON.
Complete an Application for Enrollment by clicking the “New Student Enrollment” button below.
- Once your application is approved, you will receive an email from the school with instructions on how to create a parent PowerSchool account. If you have not received an email within one business day, it may be in your junk/spam folder. If not, please contact the registrar.
- Create a parent PowerSchool account .
- In PowerSchool, complete the online registration forms A - I.
- Provide proof of residency within District 121 boundaries.
- Provide a copy of your student’s birth certificate.
- Provide a copy of your student’s health and immunization records.
- Provide a copy of any court paperwork/custody arrangements (if applicable).
- Pay registration fees or make payment arrangements (2025-26 fees are $217).
Once your registration forms are complete, the registrar will request records from your student's previous school.

Affidavit Information
*PLEASE NOTE - Starting with the 25-26 school year, affidavits must be signed by both the parent and the District resident in the presence of a Notary Public to be valid.*

The School District reserves the right to evaluate the evidence presented when enrolling with an affidavit. Completing an affidavit does NOT guarantee admission to District 121. A person who knowingly or willfully presents to the School District any false information regarding the residency of a student for the purpose of enabling that student to attend any school in the District without the payment of a nonresident tuition charge is guilty of a Class C misdemeanor (105 ILCS 5/10-20.12b(f).
*PLEASE NOTE: Affidavits must be signed in the presence of a Notary Public and are valid for ONE school year*
Residency Affidavit
In the situation that the parent(s)/guardian(s) cannot provide one item from category one in their name AND are residing with someone else, a Residency Affidavit Form must be completed, notarized and submitted along with FIVE (5) current items to prove residency every year.
The five items MUST include:
- THREE (3) items from the in-district resident you are residing with:
- At least ONE of the following:
- Lease Agreement (with everyone’s name listed as occupant or tenant)
- Mortgage Agreement
- Lake County Property Tax bill
- And TWO (2) of the following documents (all items must be current):
- First page of a utility bill: electric, gas, water, cable, sewer (bill should be dated within the last 60 days)
- Renters/Homeowners insurance bill
- Driver’s license/State ID
- Vehicle or Voter registration
- Current public aid card or current letter from DHS, SNAP, TANF or Medicaid confirming benefits
- At least ONE of the following:
- TWO (2) different items under the parent(s)/guardian(s) name:
- Substantial mail such as a cell phone statement, bank statement, car insurance bill, medical statement, driver’s license, vehicle registration
In Lieu of Lease Affidavit
In the situation that the parent(s)/guardians(s) do not have a signed lease with the homeowner AND the enrolling family does NOT share the home with the homeowner you will need to complete the In Lieu of Lease Affidavit Form and submit it along with TWO of the following documents (all items must be current):
- First page of a utility bill: electric, gas, water, cable, sewer (bill should be dated within the last 60 days)
- Renter’s/Homeowner’s insurance bill
- Driver’s license/State ID
- Vehicle or Voter registration
- Current public aid card or current letter from DHS, SNAP, TANF or Medicaid confirming benefits
Guardianship Affidavit
This form establishes a child’s residency in the School District when the child is not living with a natural or adoptive parent. This must be completed by the individual who has assumed custody of the child and notarized.
*** Important Note: All families registering using an affidavit must provide a new affidavit AND supporting documentation every year.***
El Distrito Escolar se reserva el derecho de evaluar la evidencia presentada al inscribirse con una declaración jurada. Completar una declaración jurada NO garantiza la admisión al Distrito 121. Una persona que, a sabiendas o intencionalmente, presenta al Distrito Escolar cualquier información falsa sobre la residencia de un estudiante con el propósito de permitirle asistir a cualquier escuela en el Distrito sin el pago de un cargo de matrícula de no residente es culpable de un delito menor de Clase C (105 ILCS 5/10-20.12b(f).

Declaración jurada de Residente

Declaración jurada en lugar de arrendamiento
En la situación en que los padres/tutores no tengan un contrato de arrendamiento firmado con el propietario & la familia que inscribe NO comparte la casa con el propietario, deberá completar el en lugar de formulario de Declaración Jurada de Arrendatario (Lieu of Lease Affidavit) y envíelo junto con DOS de los siguientes documento (todos los documentos deben estar actualizados):

Declaración Jurada de Tutela
Este formulario establece la residencia de un niño en el Distrito Escolar cuando el niño no vive con una familia natural o padre adoptivo. Esto debe ser completado por la persona que ha asumido la custodia del niño y notariado..

Step 1. Contact your previous school to start the withdrawal process.
Step 2. Visit the registration pages on the WTHS website for information.
Step 3. Gather items needed for enrollment (proof of residency, health documents, birth certificate, custody
documents, etc.)
Step 4. Complete the Pre-Registration Application for Enrollment. You will receive an automated email from
PowerSchool with instructions on how to create your parent PowerSchool account and complete the
online registration forms. If you don't see this email, please check your spam/junk folder.
Step 5. Create your parent/guardian PowerSchool account.
Step 6. Log into PowerSchool, click on Forms, then the Enrollment tab and complete online registration
screens A - I.
Step 7. Prepare for counselor meeting by looking over course selections.
Step 8. Pay fees and add money to your student's lunch account by contacting the bookkeepers
