In order to comply with Illinois School Code, Section 27-8, it is mandatory that all freshmen students, students new to high school and transfer students, who were not previously examined for ninth grade, have a physical examination along with verification of the minimum immunizations on file before starting school.

- A sports physical may not be used as a school physical.
- You must complete and sign the health history portion of the physical, which will be verified by your health care provider.
- Request that your health care provider completes the physical and immunization sections.
- The immunization section needs a signature from a health care provider.
- The health examination portion requires the signature of a physician, physician assistant, or nurse practitioner.
