Bridges Transition Program
The Bridges Transition Program, housed at the O'Plaine Campus, addresses employability and/or daily living skill deficit areas for special needs students through age 22. The program is designed to further employability and daily living skills to increase independence. The program was created with a results-oriented process focusing on improving the academic and functional achievement to facilitate the student’s movement from school to post-school activities and provide connections to postsecondary education, vocational education, integrated employment including supported employment; continuing and adult education.
An initial “Success” meeting with the Bridges Transition Case Manager must take place for all students and their parent/guardian after the student has been referred to the program. All Bridges Transition Program documentation including a Student Success Plan must be complete before students can begin classes.
