Residency Affidavit - A person seeking to enroll a child should use this form as evidence of residency when he or she cannot produce a lease, purchase property agreement, or real estate tax documents AND share a home with a district resident.  To use a Residency Affidavit, the parent/guardian must complete page one of the affidavit and provide two documents showing their name and the in-district address.  The homeowner must complete page two of the affidavit and provide the three items required to establish residency.  The School District reserves the right to evaluate the evidence presented; completing this form does not guarantee admission.

Residency Affidavit in Spanish - Una persona que busque matricular a un niño/a debe usar este formulario como evidencia de residencia cuando no puede producir un contrato de arrendamiento, compra de propiedad u otro documento similar – otros documentos también serán requeridos para establecer residencia.  El Distrito Escolar se reserva el derecho de evaluar la evidencia presentada; completar este formulario no garantiza la admisión.

Guardianship Affidavit - This form establishes a child’s residency in the School District when the child is not living with a natural or adoptive parent. It must be completed by the individual who has assumed custody.

Guardianship Affidavit in Spanish - Este formulario establece la residencia de un niño en el Distrito Escolar cuando el niño no vive con la persona que ha asumido la custodia.

In Lieu of Lease Affidavit - A person seeking to enroll a child may use this form as evidence of residency when a signed lease is unavailable AND the enrolling family does NOT share the home with the homeowner.  To use an In Lieu of Lease Affidavit, complete this form and have the landlord sign it.  Return the form with two additional documents from Category II of our proof of residency document (English  Spanish) to the building registrar.  The School District reserves the right to evaluate the evidence presented; completing this form does not guarantee admission.

In Lieu of Lease Affidavit in Spanish - Una persona que busca inscribir a un niño puede usar este formulario como prueba de residencia cuando NO comparte la casa con el propietario.  Para usar una declaración jurada en lugar de arrendamiento, complete este formulario y documentos adicionales de la categoría II de nuestro documento de prueba de residencia (English Spanish) al edificio para evaluar la evidencia presentada; completar este formulario no garantiza la admisión.
