  • Students who are transferring to our District ( not coming from a feeder school) will have 30 days to turn in a complete physical.    

  • Required Immunizations

    (1) Tdap

    (4) DTP

    (3) Polio ( if all same type of vaccine)

    (2) MMR

    (3) Hepatitis B

    (2) Varicella

    (1) Meningococcal conjugate ( MCV) age 11

    (1) Meningococcal conjugate (MCV) age 16 or older

     Physical has to be on a State of Illinois Certificate of Child Examination form.
